New Patients

All patients will need to fill out the registration form and medical questionnaire, prior to being able to book an appointment. You can obtain both of these forms from the surgery reception. 

You may be asked to make an appointment for a new patient check with the Practice Nurse and/or a Doctor at a later date. A practice leaflet will also be available when you register.

Please Note:

Filling in a form does not guarantee registration as we may, however, the surgery may refuse your application to join our list if:

  • You are outside the practice boundary

  • You have previously been removed from the practice list,
    - for example due to an irreconcilable breakdown in the relationship with a member of the practice team.

Please ask one of our receptionists to check if you are living within our practice boundary before filling out a registration form.

 

 


Allocated Named accountable GP

We are required to allocate a named accountable GP to each patient. As a new patient you will be assigned and informed of your named allocated GP. This is a contract requirement and does not prevent you from seeing the GP of your choice at the practice.

If you have a preference as to which GP you are allocated to, we will make reasonable efforts to accommodate this request.
